Fall Program Registration is open to all children born on or between August 1, 2000 and January 31, 2009
Applications must include an $80 Fall Registration fee.
Refund requests received at email before August 2 will be honored. No refunds after August 2, 2013
Siblings in the same division will be placed together. Placement requests with other players or specific coaches
will not be accepted.
Fall Program Registration Instructions
♦ All players must register online at www.eayso.org. Select Region 8001. Print and sign application.
♦ Players who used eAYSO to register for our 2012 Fall or Spring Program should log on to their account with their
username (email) and password.
♦ Include a copy of proof of player DOB
♦ Include an $80 check or money order payable to AYSO Atlanta Region 8001
♦ Application, copy of proof of DOB, and $80 Player Registration fee must be returned to our Pre-registration Events
on May 4, 11, or 18 or mailed to AYSO Atlanta, 1072 Casitas Pass Road #345, Carpinteria, CA 93013 postmarked
before May 22, 2013 to qualify for discount.
